Job summary

We are looking for a skilled and motivated Band 6 Mental Health Nurse to join our Spoke Team within the Community Treatment Team. This post offers a dynamic and rewarding mix of community and hospital in-reach responsibilities, with a focus on delivering high-quality, recovery-oriented care.

In this role, you will provide care coordination and nursing support to service users with complex mental health needs across both community and inpatient settings. You'll play a crucial part in ensuring smooth transitions between hospital and community care, attending Mental Health Act tribunals, preparing professional reports, and working closely with multidisciplinary teams.

You will need to be clinically experienced, solution focussed and competent in working with individuals, their families, and carers, in a community setting. You should have a collaborative and enthusiastic approach as well as the passion and commitment to develop and promote recovery-based interventions for adults with mental health related needs.

Main duties of the job

  • Deliver high-quality, person-centred mental health care in both community services
  • Undertake comprehensive risk assessments and care planning
  • Attend Mental Health Act tribunals and professionals' meetings as required
  • Prepare formal clinical reports for tribunals
  • Work collaboratively with inpatient teams to facilitate discharge planning and continuity of care
  • Provide support and input during mental health crises and implement robust risk management plans
  • Mentor and support junior staff, students, and colleagues
  • Liaise effectively with GPs, social workers, carers, and other service

Qualification

Essential

  • Professional Qualification and current Professional Registration as, a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist
  • Mentor Training.
  • Evidence of post-registration training in clinical/ psychological interventions / skills relevant to the post.
  • Proven track record of post registration clinical experience relevant to the post.
  • Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults) and Child Protection.
  • Up-to-date knowledge/experience of relevant legislation.

Desirable

  • Knowledge of evidence based treatments for psychosis and non psychosis

Experience

Essential

  • Awareness and understanding of professional accountability, NMC standards and other local and national initiatives.
  • 12 to 24 month band 5 qualified nurse experience dependent upon service need.
