Job summary

An exciting opportunity has arisen to join the Mental Health Practitioners working in Sunderland Primary Care GP surgeries. This is an established team working in collaboration with our GP partners as part of Community Transformation.

The post holder will need to be flexible in their approach, to respond to the needs of the service and demands of a busy primary care network. The role consists of providing assessment and brief interventions, who are presenting with a range of mental health needs to their GP.

This is service allows a seamless interface between both NHS and non NHS services including drug and alcohol services and other professions.

We are looking for applications from candidates who have experience of holistic assessment and providing brief psychological interventions. The ideal candidate will be proactive and dynamic with experience of working autonomously in a community setting, including managing risk management.

This is an exciting time to work within a community service as we continue to work in partnership with other agencies to meet the need for our populations.

Main duties of the job

To provide holistic assessment, brief intervention and signposting to service users who present to their GP with a range of mental health conditions.

Interface with partners including secondary care and third party organisations.

Engaging within MDT and Multiagency working

Education and Qualification

Essential

  • Professional Qualification and current Professional Registration as, a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist
  • Mentor Training.
  • Proven track record of post registration clinical experience relevant to the post.
  • Evidence of post-registration training in clinical/ psychological interventions / skills relevant to the post.
  • Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults) and Child Protection.
  • Up-to-date knowledge/experience of relevant legislation.

Desirable

  • Post registration qualification in psychological intervention
  • Experience of working within a community team.
  • Non-Medical Prescribing

Skills and Competencies

Essential

  • Proven ability to carry out and provide comprehensive written assessments of individuals with complex mental health needs.
  • Risk Assessment and Risk Management / Safety Planning skills.
  • Demonstrate an understanding of effective & appropriate treatment options including a range of evidence based clinical approaches as identified in N.I.C.E. Guidance).
  • Demonstrate effective written and verbal communication skills.
