Job summary
We have an exciting opportunity for an appropriately experienced and qualified senior clinician to join our team as advanced practitioner within our learning disability pathways across both Sunderland and South Tyneside CYPS teams.
Main duties of the job
The successful candidate will be responsible for leading and supporting the learning disability pathway offering senior clinical support, supervision and role modelling for colleagues.
There will be an expectation to carry a small complex caseload of patients as well as upskilling and supporting the wider team.
Attending, chairing and leading on MDT meetings, child protection conferences, multi agency and risk management meetings both internally and with external partners.
Supporting on developing and delivering PBS care plans and other behavioural support plans appropriate for the patient population
Providing clinical supervision to colleagues
Supporting the team and clinical manager with service improvement initiatives and ensuring high quality care is delivered.
Care coordination
Supervision
Senior practitioner support to the wider team
Supporting with and leading on complaints and After Action Reviews when appropriate
Ensuring high standards of care and treatment including upskilling staff clinically as well as ensuring adherence to documentation and record keeping standards.
Offering training and delivering presentations.
