Job summary
An exciting opportunity has arisen for an experienced mental health clinician to work as a Primary Care Mental Health Practitioner within Whitley Bay Primary Care Network (PCN).
Main duties of the job
In this role you will be working within a multi-disciplinary team; alongside other PCN roles including mental health practitioners, clinical pharmacists and social prescribers, as well as community mental health, social care, pharmacy, hospital and voluntary sector colleagues; focusing on a personalised care approach to achieve the best possible care outcome for patients.

The successful candidate will be a skilled and experienced mental health practitioner who can work independently to offer assessment and evidence based brief interventions, risk assessment for acute distress, personalised care planning and signposting or referral on to recovery and support services. Applicants should have extensive experience of working within mental health supporting individuals with psychiatric and psychological disorders live as healthy and independent lives as possible. The role will focus on those individuals who may not meet the thresholds for secondary care mental health services, but who may benefit from a comprehensive treatment plan for mental health that addresses multiple areas. This role offers an exciting opportunity to be at the forefront of community mental health transformation as outlined in the NHS long term plan.
The successful candidate must be passionate about delivering excellent patient care, and be dynamic and motivated. You must have the ability to work autonomously but also as part of a multi-disciplinary team, and demonstrate excellent leadership skills.

- Professional Qualification and current Professional Registration as, a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist
