An exciting opportunity has arisen for an experienced mental health practitioner to work in a multi-disciplinary and multi-agency primary care setting in North Cumbria.
Applications are welcome from experienced first level registered nurses - RMN, RNLD or practitioners with first level qualifications- Social Worker, OT, Clinical Psychologist. The post is based at Carlisle Rural Primary Care Network (PCN) - covering Brampton and Longtown Medical Practice.
As an experienced practitioner this is an exciting opportunity to work within the primary care setting, utilising a mental health service model delivering an essential service for those presenting at GP practices with a variety of mental health problems.
The primary care mental health practitioner (PCMH ) role enables people to access responsive interventions close to home in their community. This way of working provides a person-centred approach with a focus on recovery.
The PCMH practitioner will be the main link between primary and secondary mental health services and will network with other community resources and providers to improve access to appropriate mental healthcare. There will be a good understanding of what services and resources are available to effectively signpost to a range of community-based options.
The practitioner will work collaboratively with patients, carers, and their families networking with the wider multi agency team.
Main duties of the job
The PCMH practitioner will complete specialist holistic assessment with patients supporting shared decision making including self-management. Evidence based brief biopsychosocial interventions will be provided where appropriate.
There will be a recognition and understanding of health inequality and potential impact, advice will be offered around making reasonable adjustments to ensure services are accessible.

Reception within GP surgeries can often be the first point of contact for people seeking mental health support. Referrals to the PCMH practitioner for moderate and acute mental health problems can occur at this stage as well as from the Acute Triage team made up of GP's and Advanced Nurse Practitioners in the practice.
The PCMH practitioner role works in collaboration between the PCN and CNTW NHS Foundation Trust to provide mental healthcare and treatment to patients within primary care settings. As such there are clearly defined management, clinical supervision and appraisal processes to support practice and personal development.
