Job summary

An exciting opportunity has become available to work across Sunderland in the role of Primary Care Mental Health Practitioner. The successful candidate would be employed by CNTW however would work directly into Primary Care Networks across different GP practices.

This is an exciting opportunity for a motivated and dynamic mental health professional to work alongside a multi-disciplinary team who will be integrated within a primary care setting. This post aligns with the trusts values of community transformation by ensuring a person's biopsychosocial needs are met at the very start of their journey.

The post holder should have excellent communication skills with a keen interest in service improvement and delivery. The role comprises of primarily clinical working, including the initial assessment of an individual, delivering of psychological interventions , sign positing and managing a caseload.

Applicants will be expected to build, develop and maintain strong networks and relationships across Primary Care Networks, and all other partners whilst maintaining strong links with existing CNTW Services. Main duties of the job

An experienced mental health practitioner, providing professional advice, support, and improving access to mental health care by delivering holistic assessment and evidence based care in the context of a multi-disciplinary multi-agency primary mental health service.

Act as the principal mental health conduit between primary and secondary mental health services. Improve the quality of patient care and service within primary care.

Practice autonomously in accordance with the local and Trust Policy, Protocols, Standards and Guidelines.

Identify health inequalities and give advice as to reasonable adjustments that can be made in order to ensure services are accessible.

Disseminate information about best practice and lead on the implementation of good practice that has been developed locally and nationally in conjunction with the Trust.

Essential

  • FProfessional Qualification and current Professional Registration as, a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist
  • Mentor Training.
  • Evidence of post-registration training in clinical/ psychological interventions / skills relevant to the post.
  • Proven track record of post registration clinical experience relevant to the post.
  • Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults) and Child Protection.
  • Up-to-date knowledge/experience of relevant legislation.
