An exciting opportunity has arisen for a Band 5 registered practitioner to work within the speciality field of a Community Treatment Team based in North Tyneside CTT

Due to the area of expertise of the service, we have developed a bespoke programme to support those without experience in this specialist field to learn and develop their skills through mentoring and training on the job.

North Tyneside Community Treatment Team is a multidisciplinary service which provides assessment and treatment for people experiencing moderate to severe mental health problems. Your role will incorporate working closely with partner agencies/services in delivering effective care and treatment.

Main duties of the job

The successful candidate will be responsible for the assessment, management and care coordination of service users who have a variety of complex needs, diverse presentations, and a range of mental health conditions.

In collaboration with the Service User and their families, the post holder will facilitate formulation which then informs a Recovery and Wellbeing focussed Care Plan, ensuring regular reviews to monitor progress and adapt the Care Plan as necessary.

They will work as a member of the Multi-disciplinary Team ensuring that high quality individualised care is delivered which ensures safety and a commitment to promoting resilience, recovery and wellbeing and maximising independence.
