Job summary

The West Northumberland Community Treatment Team are looking for a qualified Community Mental Health Practitioner, either Band 6 or a Band 5 with experience and a wish to progress into a band5/6 development position. If you want the opportunity to become part of a warm, friendly MDT with a great range of skills and experience, then this post for you. Main duties of the job

We are looking for someone who is committed to providing compassionate care that is based on empathy, kindness, respect and dignity. You will assist in the delivery of therapeutic clinical based interventions and activities which meet individual needs embracing equality and diversity. You will engage in regular family/carer contact to support and encourage collaborative decision making

Be responsible for the Assessment and Management of service users who have substantial and complex needs.

To consider Carer needs and ensure they are involved in Care Planning where appropriate

To deliver evidenced based Clinical and Psychologically based Interventions.

To work as a member of the MDT ensuring that high quality individualised care is delivered which ensures safety and a commitment to promoting recovery and wellbeing and maximising independence.

Coordinate Treatment Planning & Interventions, Reviews and Discharge Planning.

Work flexibly to meet the needs of Service Users across pathways which may include working extended or flexible hours.

To have access to a car for work purposes and a UK driving licence.

To support the overall running of the team and service, by feeding into MDT meetings and cover role of duty worker.

Education and Qualification

Essential

  • First Level Registered Nurse RNMH, RMN, RNLD or equivalent first level qualification e.g. Social Worker, OT, Clinical Psychologist, other.
  • Mentor Training.
  • Evidence of post-registration training in clinical/ psychological interventions / skills relevant to the post.
  • Proven track record of post registration clinical experience relevant to the post.
  • Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults) and Child Protection.
  • Up-to-date knowledge/experience of relevant legislation.

Desirable

  • Post Registration Qualifications in Psychological Interventions
  • Experience of working within a community team.
  • Non-Medical Prescribing

Knowledge and Experience

  • 12 - 24 months minimum experience post registration.
  • Awareness and understanding of professional accountability, NMC standards and other local and national initiatives.
