This is an exciting opportunity for an experienced and motivated health or social care professional to take on a key leadership role within our integrated Transfer of Care Hub. Working across Cumberland Infirmary, West Cumberland Hospital and community hospital sites, the post holder will provide senior operational leadership to ensure safe, effective and timely patient discharges in line with the nationalHospital Discharge and Community Support guidance and the Home First principles.

As Transfer of Care Hub Manager, you will lead multi-disciplinary teams across health and social care, supporting staff to remove barriers to discharge, improve patient flow and reduce delays. The role demands strong leadership, high-level problem-solving and the ability to work collaboratively with system partners including Adult Social Care, the ICB, CHC, community and voluntary sector services.

This post is pivotal to supporting the system through periods of pressure, particularly winter escalation, and to driving forward quality improvement and performance across all discharge pathways. You will report to the Transfer of Care System Leader and be part of a dynamic leadership team committed to delivering seamless transitions of care, improved patient outcomes and sustainable system flow.

Main duties of the job

o Provide visible, proactive operational leadership to the Transfer of Care Hub teams across acute and community hospital sites.o Lead the coordination and delivery of safe, timely and person-centred discharges in line with Discharge to Assess and Home First principles.o Act as the senior escalation point for complex discharge issues, supporting multidisciplinary teams to resolve barriers and maintain patient flow.o Oversee performance monitoring and reporting of key discharge metrics including No Criteria to Reside (NCTR), Delayed Length of Stay (DLOS) and pathway conversion rates.o Work closely with Adult Social Care, the ICB, community and voluntary sector partners to ensure an integrated, system-wide approach to discharge planning.o Lead and support staff development, supervision and appraisal across multiple sites, promoting high standards of professional practice.o Contribute to service improvement, data quality and performance assurance to reduce delays and optimise the use of available bed capacity.o Support surge planning and operational response during periods of system pressure, including weekends and winter escalation.
