Band 6 Cardiology Specialist Nurse 22.5hrs a week available - Permanent
The Cardiology Nurse Specialist will form part of a cross-site team created to enhance the care of patients with newly diagnosed or chronic heart failure and patients with cardiovascular conditions.
Also provide expert nursing care for patients with ischaemic, congenital and valvular heart disease undergoing potentially high risk invasive and non-invasive procedures.
- Contributing towards the management and development, of both the inpatient and outpatient heart failure/cardiovascular nursing services in north Cumbria.
- Reducing unnecessary hospital admissions and re-admissions by contributing to timely and appropriate assessment and management of newly diagnosed and chronic heart failure patients and patients with cardiovascular conditions.
- Managing an individual patient case load and advocating seamless care for patients with heart failure across north Cumbria in both the hospital and community setting.
- Contributing towards the delivery of speciality specific education and training for patients, their carers, healthcare trainees, and all members of the MDT across primary and secondary care.
- Participating in data collection for the national heart failure audit and any other relevant local, national or international audits or registries.
- Provision and delivery of expert nursing care for patients with acute and chronic heart disease undergoing high risk invasive/non- invasive investigations.
Main duties of the job
The successful candidates will be part of a wider team of Cardiac Specialist Nurses increasingly working collaboratively across primary and secondary care. We are looking to recruit dynamic, adaptable and committed individuals who will contribute towards the management and development of both inpatient and outpatient cardiac nursing services in north Cumbria implementing national best practice.
A key part of the role will be to provide nursing care for patients with acute and chronic heart disease to improve patient flow in line with current NICE recommended guidelines for Heart Failure. This will include an in reach service to ward areas and clinics in an outpatient setting.
Working across our two hospital sites in Carlisle and Whitehaven, the post holders will manage an individual patient case load and advocate seamless care for cardiac patients, contributing to timely and appropriate assessment and management of newly diagnosed and chronic cardiac patients
