Senior Occupational Therapist - Your Health Matters

Job summary

This is an exciting opportunity for an innovative Occupational Therapist to combine the role of an ICC Occupational Therapist with the development and of an emerging service within Keswick and Solway working alongside the Band 7 lead OT.

This post is part of the Additional Roles Reimbursement programme, which is a term used to refer to a range of new, centrally funded roles which allow PCNs to further widen the multidisciplinary teams to provide more integrated health and social care services locally. The post will be hosted by North Cumbria Integrated Trust funded through the PCN.

The Anticipatory Care team (ACT) will aim to provide proactive personalised care to patients within our ICC/PCN, Personalised Care and Support planning is a series of facilitated conversations in which the person, or those who know them well, actively participates to explore the management of their health and wellbeing within the context of their whole life and family situation.

Main duties of the job

Anticipatory Care Planning (ACP) is a person-centred, proactive approach to help people consider what is important to them and plan for their future care. ACP involves conversations between individuals, their families, carers, and professionals. It helps people communicate goals and preferences, and gives them opportunities to consider realistic options and plan for future changes in their health and care that can be recorded, shared and reviewed
