Clinical Support Worker, Virtual Ward

Job summary
We have a unique opportunity for you to cover maternity leavefor virtual ward.
We are looking for experienced, motivated clinical support worker higher level Band 3 health care assistants with a passion to deliver exemplary care to patients in their own home as an alternative to hospital based care . Supported by the clinical team including specialists in all branches of the Virtual ward, you will enjoy a varied working day, providing individualised care to patients at acute hospital sites and community settings including the patients own home.
You will provide the support and care required to enable patients and their families to reside on a virtual ward including helping them with technology.
Main duties of the job
You will work alongside the Virtual ward Clinicians to deliver effective and high quality services for patients on the Virtual ward across North Cumbria.
You will have direct contact with patients, their families and carers to deliver personalised monitoring, care and education. You will also participate in remote monitoring and telephone and video assessments. You will provide, receive and process routine information. You will participate in regular MDT meetings.
The post holder will work under the direction of a registered professional/Person in Charge; they will spend the majority of their time working unsupervised delivering personalised care and nursing duties in the patient's home, hospital or hub.
