Job summary

We are looking for an enthusiastic individual, interested in developing the skills to keep our population healthy and independent.

We are a supportive friendly and innovative rehabilitation team delivering rehabilitation in peoples own homes and via our staying steady group unique to Eden ICC.

Therapy/ Rehabilitation assistants play a key role in facilitating early discharge from hospital or preventing unnecessary admission this may include exercise programmes or equipment provision alongside participation in a range of health promoting groupwork.

Full training will be provided to the successful post holder and we welcome informal visits to the team or any questions you may have about this post

Community visits are undertaken across the beautiful Eden Valley.

Main duties of the job

A Therapy/Rehabilitation Assistant in a multi-disciplinary team is expected to work with other health and social care professionals to develop the ethos of multi-disciplinary, integrated care and seamless services ensuring that patient care is delivered to the highest possible standard.

As a Therapy/Rehabilitation Assistant reporting to a registered health professional, the post holder will implement rehabilitation plans for patients in the community and other settings, observe the patient's condition, providing direct care with a rehabilitation and therapy focus. This may include supporting patients with personal hygiene and activities of daily living.

Flexible working will be required to meet the needs of the patient group.

The ability to work over 7 days is a requirement of the post. The hours of work will be on a rota covering a shift pattern between the hours of 8 a.m. and 8 p.m.

Continuing professional development is actively encouraged and as part of the team you will have the opportunity to take part in the process.

Car driver / able to travel independently of public transport around the geographical area is essential.
