Job summary
This is an exciting opportunity to join the Diabetes team at North Cumbria Integrated Care NHS Foundation Trust (NCIC) as a band 6 In reach/outreach Specialist Diabetes Nurse. This relatively new role within the Trust aims to increase and develop the provision of diabetes support and staff training across both inpatient and community settings.
The post holder will be an integral member of the Diabetes Service specialist team, providing care, treatment and education to patients living with diabetes and their families. This will involve working closely with the community network of health care professionals to deliver an integrated package of care between hospital and primary care in particular to empower a patient's self-management with their insulin.
The aim of the role is to support admission avoidance through early reviews andprovision of 1:1 patient education within a patient's own place of residence and also to facilitate early discharge from acute and community hospital settings by providing continuity of care. This involves close working with District Nursing Teams for complex case management and also community hospital wards.
Main duties of the job
To demonstrate a basic knowledge and develop specialist knowledge andclinical expertise in the nursing management of people with diabetes mellitus.
- To support the principle of self-management for people with diabetes and sign-post people to information and support to encourage informed decisionmaking about living with diabetes and managing life events (e.g. peerreviewed structured education programmes).
- Support the person with diabetes in setting realistic goals and in theachievement of those goals. This is often in unique circumstances.
- To support and provide training to NCIC staff in all settings in relation to management of diabetes. This may be acute wards, residential homes, district nursing teams or GP practice.
- Establish, maintain and titrate insulin regimes, and where appropriate review for discontinuation of insulin.
- Demonstrate competency in the development of individualised management plans for a patient with diabetes in inpatient and community settings.
- Be aware of national and local guidance and training requirement on insulin safety.
- Demonstrate awareness of the importance of daily foot checks in those with poor mobility, the frail and the bedbound.
- Be familiar with the person with diabetes' treatment regimen and device or delivery systems.
