Job summary

Come and join our friendly, growing Pharmacy Homecare team !

  • Assist in the development, management and monitoring of the delivery of medicines to North Cumbria Integrated Care NHS Foundation Trust patients through Homecare services
  • Provide clinical verification of prescriptions and work with clinical specialties to ensure patients receive safe and timely supply of medicines.
  • Provide a visible role in the provision and development of Homecare services, and act as a point of contact for the management of High Cost medicines.
  • Help to manage, support and train pharmacy staff involved in the delivery of the Homecare medicines management service for patients of the Trust
  • Use knowledge of High Cost medicines, their commissioning, management, reporting and approval, and disseminate this in the Trust as needed, to improve the governance of such medicines.
  • Manage cost-effective medicine switches to ensure best value for money in the NHS.
  • Practice as or work towards being a pharmacist independent prescriber in line with Trust policy and the needs of the Trust.
  • Support the Pharmacy team to purchase high cost medicines in a safe and cost-effective way.
You will be encouraged to retain patient-facing clinical duties within your role, depending on your area of expertise or interest. You will be supported to develop in your career pathway by appropriate mentorship.

Main duties of the job

Supervise, plan and direct the homecare team on a day-to-day basis under the direction of the lead pharmacist and lead technicians for homecareWork with the lead pharmacist on the planning and implementation of strategic goals for the Pharmacy Homecare team.Lead in the implementation, monitoring of compliance with, and updating of Trust policies relating to the homecare service.Review relevant Trust policies and procedures related to homecare to ensure compliance with relevant professional and clinical standards, including the Royal Pharmaceutical Society Professional standards for Homecare Services and the Hackett report.Develop clinical excellence in services provided for patients, such as homecare consultations for initiation and switching of therapies and ongoing patient support.Develop and implement procedures for clinical screening of homecare prescriptions including document approval and control, and provide training to clinical pharmacists in the clinical screening of prescriptions of medicines supplied through the homecare route.Carry out clinical screening of prescriptionsWork closely with the Commissioning and Finance teams to develop processes to manage funding challenges arising from the use of homecare medicines.
