Job summary
A rare opportunity has arisen to join our community based Heart Failure Specialist Nurse Team.
We are looking for a motivated and committed Heart Failure Specialist Nurse with the passion for improving the experience and clinical outcomes of our patients, and who inspires others to provide excellent patient care to join our community based team.
You will be an experienced cardiac nurse and either have or are looking to develop non-medical prescribing, clinical assessment and advanced communication skills
To work with primary and secondary care to reduce the need for unplanned admissions to hospital and improve patients quality of life through prevention, treatment, education and early detection of deterioration
Main duties of the job
- The post holder will work within the team to manage patients with a diagnosis of Left Ventricular Systolic Dysfunction through clinical expertise and advanced education and communication skills
- Use highly specialist skills and expert judgement to evaluate and prioritise referrals
- Plan, manage and organise a clinical caseload of patients, involving phone consultations, home visits and clinic attendance as appropriate for each patient. Whilst remaining responsive to the fluctuating and unpredictable needs of patients and carers
- Discuss complex patients with Consultant led MDT and cardiac device meetings
- Plan and instigate discharge from heart failure specialist team caseload, as appropriate. Formulating individualised care plans for patients and GP's on discharge as per British society for heart failure guidance
- Collect, collate, analyse, evaluate and report information using IT skills and maintain accurate patient records
- Ensure the service is accessible, flexible and responsive to the needs of the health care community
- Support any relevant service development projects
- Establish, evaluate and sustain standards of care through professional leadership
- Organise and implement education and teaching sessions for the team and other healthcare professionals, such as community teams, cardiac nurses and GP surgery colleagues
