Job summary
Health Care Assistant - Isel Ward
A collective of 12.5 hour shifts available, working up to 37.50 hours per week.
Ensure that the organisation and leadership of nursing and midwifery services supports the delivery of the highest standards of patient care in the pursuance of the Trust's objectives. The core nursing team is responsible for ensuring that effective systems of work are in place to ensure the safe delivery of evidence based patient care. Support effective patient flows and management of resources in the allocated clinical area in the Trust.
Main duties of the job
- To assist and contribute to the initial and ongoing assessment process in conjunction with a registered professional , this includes general observations and mental Test Scores, they will contribute to the evaluation of the impact of care plans
- To develop skills and experience at carrying out delegated duties in respect of the physical and psychological needs of the patient
- To contribute to the planning and delivery of goal orientated care plans under the direction of a registered professional. This can be for a range of patients with complex needs
- Take responsibility for reporting progress or failure to progress in treatments, to enable to MDT to review and plan further care interventions
