Job summary

North Cumbria stroke services are delighted to offer an exciting opportunity of a, Part time 30 hours post, to be involved in implementing a new North Cumbria Community Stroke Discharge and Support Team. New developments has meant there is an opportunity to pilot community stroke rehabilitation and support services for all stroke patients leaving acute care in North Cumbria. This role will provide secretarial support to the full team.

The community rehabilitation service provides specialist assessment, rehabilitation and advice for patients, families and carers. Emphasis is placed on the provision of a holistic care package tailored to the individuals needs with the aim of developing maximum independence and hence improving quality of life.

The new multidisciplinary team will allow specialist case management and therapy work to continue for up to 6 weeks post hospital discharge in peoples own homes, residential or nursing homes, liaising with existing community services such as neuro physiotherapy, stroke speech & language therapy teams. They would also complete 6 month stroke reviews, and ensure that where needs for further therapy are identified the specialist team will work closely to develop rehabilitation programmes for this and upskill existing community services who can support with implementing these programmes. This seamless provision ensures equity of service, patient choice and high standards of personalised care.

Main duties of the job

Please note that secondments will be considered and the base can be negotiated at the interview stage

  • To provide a full secretarial service to the North Cumbria Community Stroke Discharge and Support Team
  • To work within a stroke interdisciplinary team to provide high quality specialist care to patients following a stroke in their own home/communities, in order to allow patients to achieve their optimal level of independence and maximum rehabilitation potential.
  • To work collaboratively across the North Cumbria Integrated Stroke Service to ensure a seamless provision of stroke services across the acute hospital, inpatient rehabilitation and community teams.
  • Provide transcription, administrative and secretarial support service to the team.
  • Use own initiative, work under pressure, keep to strict deadlines and have excellent organisational, interpersonal and communication skills.
  • Contribute to service development and data collection within North Cumbria Integrated Stroke Services.
  • The role will involve travelling across the North Cumbria region as well as utilising technology to offer services virtually
