Job summary
Working within GP practices and working closely with the wider ICC team, the Wellbeing Activator will work within the Wellbeing Service supporting the Senior Living Well coach, and Living Well Coaches, providing training, and devising, developing and implementing Wellbeing Groups for individuals who present with Long Term Conditions (LTCs), Frailty and psycho-social needs engaging them to set personalised goals to support self-management of LTCs and associated psycho-social difficulties. This post will also provide line management to the Senior Living Well Coach and group supervision for the Living Well Coaches.
