Job summary
The post holder will work within the Department of Physical Health and Rehabilitation Psychology (PHRP). Within this service we have a number of pathways of care including:
- Locality, for long term conditions (LTCs) working with the Wellbeing Service.
- Persistent Physical Symptoms Service (PPSS) for patients with persistent pain and chronic fatigue syndrome (CFS).
- Neuropsychology and Stroke Wellbeing and Rehabilitation
- Familiar Faces, for patients who present frequently within the health care system.
This post-holder may be required to work across different service areas. Rotation between services, in line with agreed competencies may be offered with appropriate supervision and development. Initial service allocation will depend on current experience.
The service is considered locally and nationally as an exemplar service, offering new and innovative models of psychological care.
Main duties of the job
The post holder will become part of a team providing psychological care to patients, working directly and indirectly, providing:
- Direct care for complex cases, including assessment and therapy, working with patients and their families.
- Training in a range of psychological skills including Cognitive Behavioural Therapy (CBT).
- Consultancy to teams, MDTs, GPs etc.
- Providing group interventions with other AHPs.
The post holder will:
Work with MDT colleagues and other health workers, social services, local authority and education colleagues as appropriate.
This will include frequent working with medical consultants, therapy staff, and clinical nurse specialists (CNS), with a role in forming rehabilitation structures in the community by working with GPs etc.
They will also:
- Work closely with other PHRP pathways.
- Liaise with other NCIC/CNTW services, for example First Step, Unity, CMHART
- Liaise with Secondary Acute care teams
- Liaise frequently with ASC colleagues and third sector care and support services
