Job summary
Growing With You is a 0-25 CAMHS team working with Camden Children Looked After, Unaccompanied Asylum Seekers and Care Experienced Young People. It is a multi-disciplinary specialist team which is co-located with social workers from the Local Authority Children Looked After and Permanency Teams.
The post holder will be part of a multi-disciplinary team that assesses mental health needs; advises on appropriate mental health provisions, risk management; care planning and provides interventions and therapeutic support to children, young people and their carers. The team has a community approach and will provide a range of interventions in an attempt to engage and support this group of young people to develop relationships with professionals.
The team promotes joint working between different agencies and works in partnership with carers and other professionals to achieve the best outcomes for children and young people, whilst maintaining professional independence.
Main duties of the job
The post holder will be required to work within a multi-disciplinary team delivering a range of outcome focused, evidence based, and complex interventions and responsible for the formulation and delivery of specialised programmes of care, working collaboratively with other clinicians and stakeholders.
The post holder will hold a highly complex caseload and develop and formulate specialised programmes of care for service users that are outcome focused and evidence based.
The post holder will communicate complex information in a highly skilled and sensitive manner to members of the multidisciplinary team, care partners and service users, who may on occasion, present with challenging behaviours.
The post holder will act as a case manager for service users being treated by other colleagues, providing oversight of assessments, treatment and discharge of patients.
The post holder will maintain a high standard of clinical records, preparing reports for internal use and for external agencies, maintaining good communications with referrers and other professionals involved.
