Job summary

An excellent opportunity has arisen for a therapist with a strong interest and experience in neurology, and a passion for community service provision, to lead the Tower Hamlets Community Neuro Team (THCNT). Based at Mile End Hospital, you will be part of a large,friendlyandhighly successfulmultidisciplinary team delivering neurorehabilitation and disability management to patients in their own homes, or on-site in the gym or clinic. The teamcomprisesPhysiotherapists, Occupational Therapists, Speech and Language Therapists, Clinical Psychologists, a Neuro Navigator and Rehabilitation Support Workers, all working collaboratively to provide high-quality, patient-centred care.

The ideal candidate will bring a genuine passion for community working andpreviousexperience in neurology, along with excellent communication skills, initiative,enthusiasmand the ability to work both autonomously and as part of the wider multidisciplinary team. There is close integrated working with a number of multi-agency stakeholders, including but not limited to, the wider Neurological service within Barts Health, other community health services, primary care services and the local authority.

Main duties of the job

To provide operational and clinical leadership to the Tower Hamlets Community Neuro Team (THCNT).

To support the team in identifying; managing & delivering a range of health care interventions in the community to support people in their own homes and/or other community settings.

To provide clinical / professional supervision to the Allied Health Professionals within professional field.

To provide line management to Allied Health Professionals and administrative staff within THCNT.

The post holder will be able to work autonomously and support the THCNT in undertaking assessments and planning care to support diagnosis, provide interventions, refer appropriately and lead the co-ordination of care, working with other Community Health Services, Primary Care, Acute services, partner agencies and key stakeholders.
