Job summary
We are looking for an enthusiastic and highly motivated Team Lead for our Patient Pathway Coordinator team in Congenital Cardiology in the Specialised Cardiology directorate. You will work with the Delivery Manager to line manage staff, improve the administrative pathway and complete some PPC responsibilities.
You will be working with consultants, nurses and the wider MDT to deliver a high quality Congenital Cardiology service. This includes transition patients from Great Ormond Street, routine follow ups, outreach clinics andurgent patients. The team lead will manage the clinics and do a forward look to ensure diagnostics and clinic utilisation is appropriate
Main duties of the job
You will work with the Service Manager/Delivery Manager or Senior Patient Care Coordinator (line manager) to ensure that administrative processes for the patient pathway meet the quality standards required both nationally and within the directorate and are adhered to. You'll support with improving the service and administrative pathways by offering prompt and professional communication with patients and other healthcare providers. You will have an understanding of the Patient Tracking List and support with managing this
