Job summary

Disabilities and the wider Learning Disabilities Team, you will act as a visible clinical expert, advisor and advocate, championing reasonable adjustments, reducing health inequalities, and ensuring our services meet national best practice standards.

You will support patients, families, carers and clinical teams through direct care, advice, education, training, and system improvement. Whilst this post will be based at Newham Hospital you will be required to work across Barts Health that will allow you to build strong partnerships with colleagues in acute, community, social care, and voluntary services. You will also contribute to local and national learning and innovation through active engagement in different improvement initiatives such as the LeDeR programme and the NHS England Learning Disability Improvement Standards.

Main duties of the job

To deliver equitable, safe, high quality services for a patients with a learning disability and autism using Barts Health Services

To deliver on-going expert clinical input for complex patients with learning disabilities (LD) and autism across all sites and all departments within Barts Health

To offer clinical teams in all settings across all sites, expert advice, support and education to make personalised reasonable adjustments and promote use of Hospital passport

To plan, organise and co-ordinate the admission and discharge process for complex patients

Deliver training to all grades and backgrounds of staff to support awareness of the needs of patients with a learning disability and autism

Work with patient advocacy groups in the community , family and carer groups, to illicit on-going feedback for improvement of hospital services in the care and treatment of people with learning disabilities and autism

To lead on regular audit, and quality improvement initiatives. Conduct research with multi disciplinary colleagues

  • Relevant Degree, or equivalent or working towards

Desirable

  • Post registration course (s) in physical health care
