Barts Health NHS Trust is recruiting for a Lead Clinical Pharmacist to drive high-quality, patient-focused services and medicines optimisation across our network. The post holder will shape and deliver outstanding patient-centred care across renal and urology wards, dialysis units, and clinics.

Key Responsibilities:

  • Provide advanced clinical pharmacy leadership for renal and urology wards, dialysis units, and outpatient clinics.
  • Lead a dedicated pharmacy team, ensuring continuity and excellence in service delivery.
  • Develop, implement, and review evidence-based prescribing and formulary guidance.
  • Optimise medicines safety and cost-effective prescribing practices, minimising adverse reactions and improving patient outcomes.
  • Collaborate with multidisciplinary medical, nursing, and allied health teams to support effective patient flow and care.
  • Manage the unit's drug budget; design and deliver medication-related cost improvement plans.
  • Oversee medicines reconciliation, development of Patient Group Directions and Shared Care Guidelines.
  • Guide safe transfer of prescribing and monitoring between secondary care and other settings.
  • Support strategic service development in line with Trust, departmental, and ICS vision and plans.

Main duties of the job

The post holder will carry out the full range of duties as the Lead Clinical Pharmacist - Renal and Urology to deliver patient focused, quality care across the network to cover the following areas: renal and urology wards, dialysis units, renal assessment units and outpatient clinics.

The post holder would be expected to lead the clinical pharmacy service in the Renal and Urology services in providing responsible, safe and cost-effective prescribing within the networks to:

Optimise patient outcomes

Minimise adverse reactions

The post also helps to deliver the broader Medicines Optimisation and Governance Strategy across the Trust. These outcomes will be achieved primarily through leadership of clinical staff, and the publication and implementation of guidelines and procedures to encourage best practice.

The post holder will support the Associate Director of Pharmacy, Deputy Head of Pharmacy & Clinical Services Manager and Pharmacists within the team to manage the professional, clinical and technical input of pharmacy into the Renal and Urology directorate.

The job description is not intended to be exhaustive and it is likely that duties may be altered from time to time in the light of changing circumstances and after consultation with the post holder.

The post holder might be required to work across the Trust at any time throughout the duration of his/her contract, which may entail travel and working at a different hospital site.
