An exciting opportunity has arisen for a motivated and enthusiastic pharmacy technician to join the cardiac team at Barts Heart Centre within Barts Health NHS Trust.
The post holder will work alongside the cardiovascular prevention primary care pharmacy team and deliver quality care to patients through efficient medicines optimisation. Main duties of the job
There is a need for close coordination with the cardiovascular prevention primary care pharmacy team and wider primary care MDT such as general practitioners, pharmacy professionals, nurses, practice managers, etc to plan workload and priorities. This involves reviewing patients notes, reviewing blood test results, triaging and counselling patients about their medicines.
The post holder will work with primary care teams to continually develop the service, while maintaining high standards and achieving best practice to meet the needs of patients and the Trust.
The post holder communicates effectively with patients to help optimise their pharmaceutical management and through liaising with other health care professionals to design, implement plans to optimise health outcomes.

- Review GP clinical systems to optimise cardiovascular medicines holistically for high-risk patients. This involves conducting searches, identifying patients and risk stratifying for long term conditions such as atrial fibrillation, blood pressure and cholesterol.
- Consult with patients regarding their medication regimes in order to optimise medicines and support their understanding of their treatment, including benefits of their medication and possible side effects. These may be patients with special needs, e.g., hearing impairment, learning disability or language translation requirements.
- Supporting adherence as per NICE guidelines.
- Supporting primary care pharmacy technicians in reviewing patients with cardiovascular disease.
- Using population health data to prioritise workload.
- Extraction of data and use of spreadsheets to analyse and present information to determine trends or highlight potential challenges.
- Participate in regular cardiac primary care team meetings and departmental meetings and contribute to hospital level meetings.
- Participate in the training of new and foundation staff, including inductions.
- Liaise with stakeholders to improve the service we provide meets the requirements of its users.
