Job summary

We are seeking to appoint 3 full-time Specialist Nurse with experience in the field of frailty to join our wonderful team based at Whipps Cross Hospital. The role will support the growth of the current frailty services and the roll out of the new Frailty Virtual Ward.

A key part of this role will be providing Comprehensive Geriatric assessment to patients on the Virtual Frailty Ward, working alongside our MDT. A key part of this role will be providing Comprehensive Geriatric assessment to patients on the Virtual Frailty Ward, working alongside our MDT.

You will contribute to the Comprehensive Geriatric Assessment & management of our patients , working closely with our multidisciplinary team. We also operate a Post Discharge Frailty Support Service which offers nurse led telephone follow up to prevent re-admission/re-attendance. We also support our Geriatricians in our Frailty Clinics. So as you can see, there's lots to do & get involved in and no 2 days are ever the same. You will work as part of a Frailty Specialist Nursing team and gain experience working in each of the frailty services we provide.

Main duties of the job

It is imperative that Barts Health NHS Trust, as a key health care partner in East London, leads the way in revolutionizing integrated care for frail older people and creates a interface services that not only best meet patient needs, but also add value in supporting patients and their careers transitioning from community to secondary care and back. The Virtual Frailty CNS will work across secondary care and the community interface and be supported by a Frailty SHO and work under the supervision of a Consultant Geriatrician on a day to day basis.

  • Registered General Nurse or equivalent
  • Prep for Mentorship course or equivalent
  • Evidence of training in capacity/DOLS/managing long term health conditions
  • BSc or working towards Knowledge of other agencies obligations and hospital discharge processes.

Desirable

  • Physical Assessment Course
  • Post Graduate Certificate in Older Peoples Care

Experience

Essential

  • Registered Nurse
  • Experience of managing and co-ordinating complex discharges
  • Experience as Sister/Charge Nurse
  • Fulfils KSF outline for Sister/Charge Nurse (Band 6) in this or comparable specialist area
  • Post registration experience including recent experience within the acute setting
  • Demonstrable understanding of guidance and legislation relating to community health services, statutory social care duties as they impact on admission avoidance and discharge.

Desirable

  • Experience of working in both community and acute settings.
  • Experience of working in Emergency Department setting.
  • Experience of working as part of a frailty team.
