Job summary

Are you a dedicated healthcare professional looking to make a significant impact in the field of respiratory care? We are seeking a highly motivated and skilled individual to join our team as a Home Oxygen Lead. This role presents a unique opportunity to lead and manage specialist care for patients aged 18 years and above on home oxygen, while contributing to service development and improvements for residents of Tower Hamlets.

The post holder will be part of a dynamic, award-winning and multi-disciplinary team. ARCARE (Adult Respiratory Care and Rehab Team) works closely with respiratory teams, across sites (occasional cross site cover may be required). This is an exciting role that supports an integrated approach in supporting respiratory patients across the various patient pathways including in-patient reviews, diagnostic (spirometry) and follow-up clinics, virtual ward, home oxygen, primary care education, and pulmonary rehabilitation. We are looking for a patient focused nurse to join our team and help the service deliver responsive and compassionate care.

Main duties of the job

  • To manage specialist care for patients aged 18 year plus on home oxygen across the pathway.
  • To perform Specialist Respiratory assessment of patients with diverse, complex cardio respiratory and multi-system pathologies across the patient pathway in a variety of settings across primary, community and secondary care. To be able to provide individualised treatment and management programme according to assessment findings.
  • To provide support and influence initiatives on oxygen provision at a local and London wide level.
  • Provide specialist support to wards, clinics and community teams
  • Maintain and develop communication pathways both within the hospital and the community to enable a fast and appropriate service for oxygen patients.
  • To be involved in formal and informal training such as clinical review sessions, peer review and in-service training
  • To supervise, educate and assess the performance of students where appropriate to a graduate standard.
  • To present findings to Stakeholders and wider in order to be involved in the implementation of service development and changes to practice
  • To have an overview/ understandingof all patients (regardless of age/ diagnosis) on the Home Oxygen Database and identify areas that would need further serviceprovision /improvement.
  • To work with the Paediatric clinical nurse specialists tooptimise home oxygen service for all patients.

To be professionally and legally accountable for all aspects of work, including the management of patients on a defined caseload. To ensure a high standard of clinical care based on current evidence and expert practice

To maintain an up-to-date list of patients, their diagnosis and details of when they were last reviewed on an up to date database.

Maintain joint working between ARCaRe, Consultants, GPs, and Oxygen provider (Air Liquide) to facilitate a coordinated approach to patients care.

Liaise with relevant teams such as respiratory, sleep and ventilation, cardiology, health care of the elderly, neurology and palliative care regarding patient care.

Ensure that Home Oxygen Order forms (HOOF) are completed accurately, dated within the last 12 months and that invoices and equipment delivered to patients is appropriate.

To provide educational visits to patients homes

Attend quarterly Air Liquide performance review on behalf of Tower Hamlets

To act as a point of reference for home oxygen enquiries

To benchmark against different HOS / HOSAR models currently in operation in the NHS

To manage clinical risk within own patient caseload.

To keep policies and procedures up to date for measuring blood gases in the community setting, specifically for housebound patients or those failing to attend Outpatient clinics. To ensure that these are kept updated with any changes when required.

To interpret findings of blood gas tests and to make safe and appropriate changes to the patients management plan,

To assess all patients who are newly started on home oxygen, within 4 weeks of referral.

To ensure there is follow up on all patients already established on home oxygen on at least an annual basis.

To promote self-care skills and be able to teach patients and carers self-care strategies, for example use of action plans and commencement of rescue medication. This will include sign posting for appropriate use of primary care services including tobacco control services and pulmonary rehabilitation.

To liaise and share care, where appropriate, with colleagues within the team.

Use advanced/specialist clinical reasoning and assessment skills to provide urgent (but not emergency) acute treatment or support in the event of an exacerbation of Respiratory Disease.

To maintain accurate and concise electronic records using EMIS-web, and to encourage all staff are doing likewise to a high standard and within the defined time frames

To carry out joint assessments with other primary and secondary health care providers to ensure comprehensive and continuous care.

To promote services which will promote patient self-care and confidence; for example generic Expert Patient Programme, disease specific Expert Patient Programmes, Pulmonary Rehabilitation and Breathe Easy patient support groups.

To collaborate with Palliative Care to provide seamless service for patients requiring advanced care planning and end of life care.

To assess patients for need for appropriate appliances and/or equipment, and competently evaluate effectiveness.
