Care Coordination Solution (CCS) and Improving Elective Care Coordination for Patients Programme (IECCPP)
The Covid pandemic has caused the elective waiting list to grow, with around 7m patients nationally now waiting for treatment. This is further exacerbated that referrals are down by c40% which means that waiting lists are likely to increase when people return to healthcare for their outstanding health issues. Barts Health Group (Barts Health) is directly impacted by this situation given its size and significance within England.
To meet this challenge, productivity is therefore a key aim of the Trust's Financial Plan in 23/24, and integral to our system Sustainability & Improvement Plan. Activity recognised through Elective Recovery Fund (ERF) and Getting It Right First Time (GIRFT) in recovering the backlog, streamlined scheduling to reduce theatre cancellations and delivering GIRFT HVLC & theatre metrics (day case rates, capped utilisation and cases per list) would all contribute toward a better productivity. Good pathway oversight is in turn critical to all three of these ambitions.

Main duties of the job
The Improving Elective Care Coordination for Patients Programme (IECCPP) is part of an NHS-wide effort to better use data to reshape health and care by giving clinicians and hospital staff the tools they need to make sure patients are treated faster. It supports Trusts and ICSs to strengthen elective care recovery.
The CCS Programme is an operationally led transformation programme working with operational and clinical stakeholders and is conceived as an integrated approach across a wide range of specialties, all with challenges and complexity in their own right. The CCS Programme works with unified management, governance and reporting mechanisms, supported by the different Trust hospitals.
The Trust is now looking to scale up the resources for this programme and supporting initiatives, bringing a Programme Manager to join our recently formed Barts CCS delivery team, to provide leadership, management and direction for the roll-out of CCS across Barts Health.The Improvement and Transformation Directorate is a vibrant and intellectually stimulating place of work where complex and demanding projects are developed and delivered for the Trust. The role requires an individual who can provide leadership, the ability to work flexibly to tight timescales and across multiple workstreams. Working with minimal supervision, the post holder will work within the Improvement and Transformation team, and across the Group Hospital sites with senior operational leads and clinical teams across the Trust. This will also include teams within ICT, Business Intelligence, Clinical Systems and Workforce Directorate.Travel to different hospital sites within the Barts Health Group and to other partner providers will be necessary.

This means the individual needs extensive experience of project management as well as an expert understanding of operational and theatres scheduling improvement to ensure that adoption of the CCS system is appropriately embedded for operational and clinical cohorts at each hospital site.As a senior member of the Improvement and Transformation team, the post holder will need to have science of improvement knowledge and excellent stakeholder engagement.
