Job summary

Barts Health NHS Trust is home to the Physician Response Unit (PRU) which was launched in 2002 with the aim to deliver safe, effective and patient-centric care in North East London by bringing the Emergency Department to the community. For 20 years the PRU has promoted a shared decision-making model which empowers patients and facilitates clinicians to identify the right care, in the right place, at the right time. This service is delivered through the tripartite relationship between Barts Health NHS Trust, London Ambulance Service (LAS) and London's Air Ambulance Charity.

PRU provides care to a multitude of patients and their families across East London. Patients and families are the focus of all the care provided hence the importance of a role focused on them.

The nature of PRU and the potential for onward referral to our community and hospital partners implies that patients and their families navigate multiple services. The patient liaison & follow-up nurse will help facilitate the process while empowering patients and families.

The role will imply a close relationship with the operational team and when possible, represent the patient and family point of view during case discussions.

A nurse with previous Community Emergency Medicine experience will be invaluable to ensure the service's smooth running and continuing development.

Main duties of the job

The patient liaison & follow-up nurse role will be non-clinical and primarily aimed at following up and liaising with patients and their families post-contact with the clinical team. The aim is to gain a deeper understanding of the impact of the service on patient care and to identify possible areas of improvement while advocating for the deliverance of companionate care. Collecting qualitative data regarding patient & family experience has a pivotal role in the service's ongoing development and will help shape pathways and improve care deliverance.

Qualifications

Essential

  • NMC registered - either Adult or Paediatric section of the Register.
  • Specialist A&E/ED course, or Equivalent
  • Immediate Life Support
  • Paediatric Life Support
  • Level 3 safeguarding

Desirable

  • Formal qualification or completion of training in leadership
  • Completion of the IPHC Trauma course

Experience

Essential

  • Previous experience in Community Emergency Medicine
  • Accident and Emergency Department experience
  • Experience in co-ordinating Resuscitation room
  • Experience in Triage/assessment of patients

Desirable

  • Have previously completed a clinical secondment with the Physician Response Unit.
  • Delivering/explaining health promotion advice to patients, their carers or parents
  • Experience in delivering quality or service improvement projects
