Job summary

An opportunity has arisen within Cancer Performance department for a motivated and dedicated Cancer Pathway Navigator.Reporting to the MDT Co-ordinator and working closely with the Management and Clinical teams, the post holder will be responsible for ensuring that deadlines are met and that there is a pro- active co-ordination of cancer patient pathways to ensure appropriate treatment is received in line with national and local targets.

The post holder will establish key working relationships with all members of the administrative and clinical teams including nursing and medical staff, allied health professional and support workers.

They will assist the MDTC in facilitating the tracking of ITT patients and surveillance tracking. They will act as a point of contact (in phone and in person) in communications with patients, relatives and GPs and other external hospital staff / tertiary referrers.

Main duties of the job

You will be responsible for providing waiting times and cancer outcomes data, monitoring and data collection (including collection and tracking of notes).

You will also be responsible for supporting weekly PTL meeting of which you will help to prepare facilitate and record outcomes of each patient via the Somerset cancer database.

As a Cancer Pathway Navigator you will ensure concise and daily tracking of each step in a patient pathway, data quality and completeness, expediting appointments/visits ensuring patients are not delayed at any point in their pathway.

You will also work closely with Wait List Teams and Secretaries and ensure regular communication with Sunderland and off-site clinics/colleagues.

Job responsibilities

To communicate with all levels of staff at Barts Health NHS Trust and other hospitals within and outside the Network, GPs and patients to obtain or verify relevant information relating to cancer waiting times and to share best practice.

To understand and communicate to clinical teams the Trusts annual Cancer Waiting Times targets and be aware of the impact on the Trust, if targets are not met.

Ensure all patients on a 62 day pathway (62 day standard, screening and upgrade) are monitored to avoid breaches.

Liaising with the radiology, pathology departments and clinicians to ensure patients are dated within the timelines as dictated by the specialty concerned.

Record accurate data on the Cancer Database (Somerset Cancer Registry)including importing diagnostic tests and pathology

Ensure the Minimum data set is received from external Trusts on patients referred for MDT discussion and/or Treatment

Ensure accurate InterTransfer dates are recorded for submission of cancer activity on to the CWT portal.

Escalate any bottle necks on the patients pathway to the Cancer Pathway Manger/s (CPMs) and/or services

To work and liaise with a diverse group of colleagues, i.e. CNSS Consultants Spr/Regs, Radiologist/Histopathologists GPS, and key stake holders within the Trust to ensure patients are facilitated through their cancer pathway.

To cross cover colleagues when they are on annual leave and throughout periods of sickness, moving between appropriate hospital sites if needed.

To assist with the preparation of MDT lists when required

Collate pathology and/or diagnostic reports for results review clinics

To liaise with referring trusts for diagnostics and treating hospitals to ensure patients are seen and treated in accordance with Cancer Waiting Times targets. Obtaining/escalating diagnostics and TCI dates and capacity issues outside of the trust where applicable.

To ensure that the Chemotherapy and Surgical validations tally with Somerset cancer registry to ensure all cancer patients are identified and captured

To attend Cancer PTL meetings for patients in the diagnostic phase

To email referral Spor forms to tertiary centers for treatment, MDT discussions and diagnostic tests

To identify patients who have DNAd their diagnostics tests or outpatient appointment and manage appropriatelyEnsure patients are followed up on an appropriate pathway once first definitive treatment has occurred.
