We are pleased to offer an exciting opportunity to join our Palliative Care Specialist Team in one of the country's largest acute NHS Trusts.
The post will be based within the existing Specialist Palliative Care Team offering ongoing support, working alongside staff to ensure that people with specialist palliative care needs are in receipt of the care they require to achieve the best quality of life possible. The post will be based at Newham Hospital.
This post holder will be highly skilled with relevant extensive palliative care experience to support the delivery of clinically effective, efficient, and high-quality care to people with complex specialist palliative care needs, ensuring that this care is both evidence based and reflects current best practice.
Using Specialist Palliative skills and knowledge you will manage a caseload of patients and their families/carers with complex needs associated with living with a life challenging illness.

Main duties of the job
The post holder will manage the provision of specialist palliative care advice and support to people affected by life limiting illnesses, and to patients who are at the end of their lives.
This will be achieved both directly through clinical practice and caseload management, and indirectly through leadership, education, research, audit, and professional activities.
The aim of the post is to maximise the quality of life of patients with advanced life limiting disease/ those at the end of their lives, and to also provide support and advice to the families/carers of such patients

- To excel and innovate in the management of clinical caseload and MDT processes both specialist and generalist palliative / end of life care
- Using advanced verbal and non-verbal communication skills, advise and empower other professionals who are directly involved in the delivery of palliative care
- Work autonomously, demonstrating innovation and resourcefulness.
- Acts as a resource of clinical excellence in the speciality of palliative care nursing, and ensure that national and local clinical policies are implemented and adhered to
- Assess, plan, implement and evaluate holistic patient care based on sound
rationale and evidence-based practice to influence and implement treatment.
- Manage triage processes and standards of response.
- Maintain and be responsible for electronic and written records in line with the organisation.
- Act as a patient advocate and influential agent of change.
- Be able to mobilise other services to contribute to patient care; working closely with the multidisciplinary team to enable best care and a shared understanding of the patients needs.
- Assess, identify, and explore complex emotional issues which patients and families face as a result of diagnosis of a life limiting disease.
- Offer skilled emotional and psychosocial support and refer on to other members of the multidisciplinary team as appropriate.
- Be an educative resource for patient and families/carers.
- Offer bereavement support to patient and families, identifying those at risk of complicated bereavement.
- Contribute to meetings to enhance patient care.
- Participate in the weekend face to face on call rota (this may involve covering 2 hospital sites)
