CLINICAL FELLOW IN GROWN-UP CONGENITAL HEART DISEASE

This one-year post is ideally suited to provide advanced cardiology training, in the management of adults with congenital heart disease. This consultant led service now provides specialist care for approximately 11000 adults, with around 600 new patients referred per annum form our sister institution Great Ormond Street Hospital and other cardiac centres around the UK.

The GUCH is based in the Bart's Heart centre, St Bartholomews Hospital, and provides long-term and often life-long care for the increasing number of adults with congenital heart disease. The service provides expert medical and surgical care within an academic environment with a range of specialities relevant to Congenital Heart patients. The service fosters a multidisciplinary approach to this specialised patient population and close links have been established with the relevant specialities throughout Barts and the London NHS trust. There is also a close co-operation and collaboration with the team of paediatric cardiologists/surgeons based at Great Ormond Street. Diagnostic and Interventional catheterisation procedures are regularly performed as well as cardiac surgical interventions therefore the experience gained is broad and varied.

The successful applicant will work with 2 other GUCH clinical fellows with supervision / training from the consultant team.

Main duties of the job

Joint Medical/Surgical MDT

The post-holder will be responsible, together with the other fellows for the organisation and presentation of cases at this bi-weekly meeting where cases requiring catheter or surgical interventions are discussed. This meeting is highly important and involves creating a powerpoint presentation of case history, angio / echo images and literature review pertaining to the case. A nominated fellow will be responsible for the triaging the listing of cases and ensuring IT / computer facilities are functioning prior to the MDT.

Admin & teaching

The post holder will be responsible, with support staff, for the maintenance of clinic database to facilitate the assessment of long-term sequelae congenital heart disease in the adult population. He/she will be responsible for ensuring that note-keeping by junior SHOs is of a high standard and is in keeping with the guidelines provided by the GUCH consultants.

Medical student and SHO teaching is also part of the weekly duties of the postholder.

The post-holder will work alongside 2 other GUCH fellows and will be responsible for the day-to-day management of GUCH inpatients both on the ward, and on the Intensive Care Unit, under the supervision of the GUCH Consultants.

He/she will participate in a daytime rota system (1:3) to reply to queries from patients, CNS team or from referring general practitioners or physicians. The postholder will be expected to attend a daily handover meeting on ward 6D at 0800 and undertake a daily ward round for inpatients and contribute to 2 consultant ward rounds per week.

Other duties include undertaking a DCC list for CHD patients and assisting in OPD with a commitment of 2-3 OPD per week. Training will be offered in the practical skills required for the diagnosis and management of medical and surgical problems in adult patients with congenital heart disease including TTE and TOE.
