Job summary

We are a pan-London Neonatal Transfer Service, that is based at the Royal London Hospital. This is one of the busiest and most intensive transfer services in the country. The service provides excellent high level specialist neonatal care to neonatal patients that are transferred between the 27 units within Greater London. We perform both emergency uplift and elective repatriation transfers.

Whilst we are separately commissioned we enjoy a close and collaborative working relationship with the neonatal service with whom we are co-located. We have a formal arrangement with several of the neighbouring transfer teams - Kent, Surrey, Sussex (KSS NTS), the Children's Acute Transfer Service (CATS) and the South Thames Retrieval Service (STRS).

You will join an enthusiastic, friendly and experienced team. We have three dedicated Consultant Neonatologists - Dr Syed Mohinuddin (Service Lead), Dr Faith Barker and Dr Nandiran Ratnavel, 12 Neonatal Nurses (band 6 &7). The nursing team is led by a Matron who is supported by a B7 PDN and Senior Nurse for PCCU & Neonatal Services. The service is supported by a service and office manager together with administrative staff. You will work within a team of 9 clinicians (senior paediatric ST trainees, senior doctors / fellows / ANNPs). In addition there are two clinical technologists that provide technical support for the specialised transport equipment.

Main duties of the job

The role aim and person specification has been developed to align with the BAPM framework 2021, this framework aims to be a resource not only for ANNPs but also employers and education providers alike, to inform service development, commissioners and healthcare providers in the planning and quality assurance of neonatal services.

  • To provide high quality care to the neonate and family
  • Contribute to the development of a high-quality neonatal service, both on the Neonatal Transfer Team and the Neonatal Unit. (This post is primarily for Neonatal Transport but with rotation to the Ward 8D NICU.
  • Practice evidence based clinical care of the neonate
  • Utilise advanced skills for the assessment, diagnosis and treatment of the sick neonate
  • Promote the role of the ANNP as part of the multidisciplinary team
  • Support the nursing team in providing excellent care to the sick neonate by supporting the education team, and contributing periodically to the nursing rota
  • Participate in unit MDT teaching programme and Transport teaching involving nursing, medical and ambulance staff
  • Ultimately the Rota will involve 6 month rotations between the Transport team and the Neonatal unit at the Royal London Hospital. The ANNP will be working on the Tier 2 Medical Rota
  • The post holder will be expected to work between emergency and elective teams possibly stepping into medical and nursing roles on the same day.

Job responsibilities

Education

To assist in the education of nursing, medical, midwifery and ambulance staff, parents and other health professionals involved in the care of the neonate.

This will include NLS, ARNI, delivery suite skills and drills training, NTS study and outreach days and NESTT/STAN course days.

Assist with the orientation/induction of new medical staff

Assist with the on-going training of the nursing staff on the NTS/ NICU

To act as a resource and mentor to appropriate staff

To be actively involved in clinical audit/research

Work together with the NICU ward ANNPs, NICU ward educators and the NTS educator towards rotational nurse working and enhancing nursing knowledge.

Neonatal Transport Duties

The duties outlined below are not definitive and may be changed in accordance with the needs of the service. The post holder may be required to carry out other duties in occasional emergencies and unforeseen circumstances.

The postholder will be allocated a shift pattern to undertake transport duties. There will be time allocated to administration, research, audit, teaching, training and leave.

When on duty for transfers, the anticipated routine for postholders will be as follows:

  1. Identify nursing team member and ambulance crews
  1. Double check (with colleague) of vehicle, transport incubator and equipment and arrange replacement, repair or additional equipment or drugs as necessary.

  1. Undertake necessary administrative duties.

  1. Freedom to join base hospital educational activities if no work, calls or transport pending.

  1. Requests for transfer will be notified to the team by the Emergency bed Service (EBS) and the team will promptly contact the referring hospital to obtain details of the babys weight and gestation, the nature of the clinical problem, and whether a bed has already been arranged. If a transport is needed, the Transport ANNP/doctor, nurse and ambulance crew will leave the base hospital as soon as possible to transport the baby. If it is not possible to immediately arrange a bed for the baby, this task will be taken on by EBS.

  1. On arrival in the referring hospital, the transport ANNP will obtain a full history and undertake an examination of the baby and its clinical status, including an evaluation of the efficacy of intensive care support. In conjunction with the nursing member of the team, the baby will be made safe for transfer, changing the babys treatment if necessary. If it is considered that a long period will be required for stabilisation, or if the baby is considered too sick for transfer, this must be discussed with the duty consultant. The transport team will remain responsible for the baby throughout the transfer until the babys care is taken over by the receiving hospital.
  2. Details of each transport shall be recorded on a database, with full details of patient demographics, hospitals involved, adverse events and outcomes. The post-holder will contribute to the production of the annual report of the transport system.

  1. The ANNP will be expected to participate in the delivery of resuscitation and stabilisation outreach training to hospitals using the service as well as the Ambulance Service.

At all times there will be a designated consultant neonatologist available for advice on clinical matters and if necessary to join the team on a transfer. Both the postholder and this consultant shall carry mobile telephones to allow communication at all times.

Neonatal Unit duties

Assess, plan and initiate the care of the sick neonate on the NICU.

Assess, plan and initiate the on-going care of the neonate on the neonatal unit, including prescribing.

To attend high risk deliveries for resuscitation, stabilisation and safe transfer of the sick neonate to the unit.

To subsequently manage mechanical ventilation, including monitoring of blood gases and altering settings as necessary.

To actively participate in ward rounds in assessing the best management of the neonate.

To perform, monitor and interpret laboratory investigations and act appropriately on results.

To request and interpret other investigations and make referrals to the relevant specialists as appropriate e.g. imaging, ECG, EEG.

Prioritising and organising own workload.

Communicating highly complex and sensitive medical information to colleagues and parents and giving support to staff and parents in such distressing occasions.

To participate and lead in nursing education days and bedside nursing.

Procedures and Clinical skills

Have the ability to undertake the necessary diagnostic and therapeutic procedures within the scope of the local guidelines, with a high degree of precision and excellent eye hand and sensory co-ordination.
