Job summary
The post holder will form an integral part of the multidisciplinary team approach in providingpersonalised care interventions to patients diagnosed with respiratory disease and their carers throughout the care treatment pathway. The Respiratory Support Practitioner will work closely with the Respiratory Specialist CNS's and Physiotherapists to support patients identified to the service by the respiratory outreach teams. They will support with all aspects of service provision, e.g.,training / equipment management, provide advice and support for the management of patients with respiratory disease. They will also support patients through the virtual ward hospital at home pathway by providing equipment and education to patients to support their discharge under early supported discharge pathway.
Main duties of the job
COPD/Asthma Bundles:1. Give advice and education to support management of their lung condition, including identifying that patients are on the correct inhaler and their technique is sufficient and selfmanagement plans are completed.2. Advice on pulmonary rehabilitation and appropriate making referrals.3. Education in recognising future signs of an exacerbation of their respiratory condition.4. Offer very brief advice on stop smoking and making referrals to the inpatient Tobacco Cessation Service.5. Collate and submit data to the National Respiratory Audit Programme (NRAP).6. Be able to perform ARTP accredited spirometry with reversibility.
Supportive discharges/Respiratory Virtual Ward:1. Take observations of patients and report back any concerns outside expected parameters.2. Visit patients in a community setting to perform observations and ensure patient is treated via the correct care pathway.3. Ordering of Oxygen for patients requiring it for discharge from hospital under the guidance of a prescribing clinician
