Job summary
We are offering an exciting opportunity to join the established and dynamic Specialist Palliative Care Team. This post will be based within the existing Community Specialist Palliative Care Team offering ongoing support, working alongside staff to ensure that people with specialist palliative care needs are in receipt of the care they require to achieve the best quality of life possible. We are seeking an enthusiastic, committed and motivated nurse with experience in palliative and end of life care to join our team. This post holder will support the delivery of clinically effective, efficient and high-quality care to people with complex specialist palliative care needs, ensuring that this care is both evidence based and reflects current best practice.
Main duties of the job
The post holder is responsible for delivering expert specialist palliative nursing advice, tosupport and facilitate high quality, individualised care to patients with complex palliative care needs. The Clinical Nurse Specialist (CNS) will demonstrate high levels of decision making, monitoring and improving standards of care through supervision of practice, clinical audit, research, teaching and by providing support to professional colleagues. The post holder will participate in a rota providing a seven day service.
The post holder will have responsibility for ensuring that her/his specialist service is reflective of the Trust Key Performance Indicators (KPI) and will support the Clinical Lead in achieving all appropriate KPI's and outcomes for Specialist Palliative Care.
The post holder will provide professional advice and leadership acting as a resource tomembers of the multi-professional team providing core level palliative care.
