An exciting opportunity has arisenfor an experienced, enthusiastic and motivated nurse to join our hard working team!

We are looking for Clinical Triage Nurses to join the Dudley Clinical Hub (Single Point of Access) offering telephone Triage, clinical advice and signposting.

Based at Brierley Hill Health and Social Care Centre, the successful applicant will join an integrated, collaboratively focused team, employing a proactive clinical practice and knowledge to provide phone triage / clinical assessment skills in order to support urgent care for patients across our health economy.

The Service is the single point of access for patients registered with a Dudley GP for all admission avoidance referrals from professionals and for patients requiring Health and Social Care Services.

The Service operates between the hours of 8am - 9pm, 7 days per week, and 365 days per year. The successful applicant will be expected to work flexibly throughout this period. The service offers extensive support and knowledge to enable patients to be managed away from the acute setting and to implement the right level of care support to achieve this.

We need applicants to be flexible, approachable and have a sound clinical evidence based background and knowledge. They will need to be friendly but assertive, and be confident in their own decision making; being able to challenge when necessary.

Main duties of the job

The Triage Nurse will provide clinical support, assistance and professional expertise to all Healthcare professionals in support of avoiding inappropriate hospital attendances and admissions, then referring onto the most appropriate service for the patient's needs. Working closely with, and acting as the referral point for the Urgent Community Response Team.

The primary objectives are to prevent unnecessary admission to acute care and to facilitate collaborative MDT management for patients closer to home. The role involves taking telephone referrals from GP's, West Midlands Ambulance Service, Community Nurses and all other Health Professionals. The triage Nurse will be expected to visit patients in their own home supported by the Medical Lead and Senior Practitioners if required. This will enable the Triage Nurse to receive further training to ensure clinical skills and competencies are maintained.

We are looking for enthusiastic and innovative individuals who possess excellent communication, customer care and IT skills. The candidates will have to be able to prioritise their own workload without supervision, have an excellent telephone manner and be able to work well on their own initiative as well as part of a team.
