An exciting opportunity for a registered Nursing Associate to join the Trust Dementia and Delirium Team. We are seeking a motivated, enthusiastic and committed Nursing Associate who has experience of supporting people living with Dementia and those experiencing Delirium. You will need to have excellent assessment, observational and communication skills and demonstrate a sound knowledge of the needs of patients with Dementia and Delirium. In return you will work within a supportive team, where you will be encourage to learn and develop new skills and knowledge
Main duties of the job
The post holder will be part of the Dementia and Delirium Team, placed within the Complex Vulnerabilities Team. You will work in partnership with the Learning Disability Team, Trust MCA/LPS Lead and Safeguarding Team to provide an integrated and comprehensive service to patients with Dementia and Delirium.
This is a dynamic and varied role and will provide opportunities to build partnerships with teams across the Trust and will require liaison with external partners to ensure timely access to appropriate services.
The post holder will focus their practice on three specific patient groups:
- People with dementia who are acutely physically ill and present with acute confusion,especially delirium, secondary to the physical health problem.
- People with dementia who are recovering from an acute physical illness.
- People with dementia experiencing delayed transfer of care but otherwise medically fit for discharge.
The post holder is expected to model safe delivery of care, and effective and compassionate dementia care. This will include working with and supporting others to develop the skills required for dementia care to become everybody's business.
