Job summary
Introduced in March 2022 our Paediatric Virtual Ward is a new initiative allowing children and young people to receive care at home with support from the specialist paediatric team. This initiative has been widely recognised as a very positive experience for children, young people, their parents and carers.
This is an extremely exciting opportunity to join our team to support our Paediatric Virtual Ward. We are looking for experienced paediatric nurses who will be able to integrate into our well-established community outreach team.
The team works in collaboration with the medical team to admit children on the virtual ward, monitor and support during their stay and undertake nurse-led discharge plans.
The ideal candidate will be highly motivated with a passion for paediatric community nursing and a commitment to service development and excellence in care. You will be required to work flexibly including weekends.

Main duties of the job
The post holder will manage a varied caseload of eligible children aged 0-18 years, (up to age 19 years when attending a special school) to provide high quality nursing services in the community for children who meet the specific referral criteria. They will support the children's ward to facilitate early discharge and prevent hospital admissions where possible.
The post holder will support the implementation of effective care packages with children/young people and their families/carers and other members of the multi-agency team, by leading the discharge from hospital into the community, including the patients who meet the criteria for the newly established virtual ward.
The post holder will be responsible for identifying and writing of guidelines, policies and standards relating to children/young people in line with government and Trust directives and monitoring standards through audit and evaluation.
The post holder may be required to work flexibly during periods of annual leave in order to provide a community service.
The post holder will participate in the education and training of nursing students on the community and will act as a role model to junior staff and other team members.
