Job summary

We are looking to recruit an experienced practitioner to join our Cardiac Assessment Team (CAT) to work autonomously across our pathways to ensure the delivery of high quality, patient focused care to those with acute cardiac presentations.

CAT leads on the management of patients. As an independent practitioner you will use your knowledge and skills to assess, risk stratify and treat patients presenting with a variety of cardiac conditions that can be managed through our Same Day Emergency Care pathway.

Working alongside colleagues within the Emergency Department you will provide clinical assessment and support in the care of a diverse and undetermined caseload with cardiac conditions. You will support in the clinical management of these patients along the pathways to the most appropriate point of care include territory centres, our own Coronary Care Unit or through the Cardiac Assessment Unit.

The successful applicant will have experience of providing care to those with acute conditions. Applicants should hold a relevant post qualification cardiac course and/or be able to demonstrate evidence of interpreting ECG's in an acute setting. Physical Assessment Skills and being an Independent Non-Medical Prescriber are desirable but not essential.

Main duties of the job

- To provide effective clinical leadership to all staff in contact with cardiac patients across the Trust.- Working as an autonomous practitioner, clinically examine and assess health and wellbeing needs of cardiac patients from a physiological and psychological perspective, and plan clinical care accordingly.- To manage a clinical caseload as an advanced level practitioner, utilising specialist knowledge and skills based on theoretical & practical experience.- Triage referrals and make onward referrals.- Demonstrate a high level of complex decision making in relation to the assessment, planning, intervention and evaluation of patient care.- Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.- To be responsible for patient's progress along the agreed care pathways, which are supported by the implementation of evidence-based practices.

Qualifications and Education

Essential

  • Registered Nurse (active registration on Part 1 or 2 of the NMC register.) or AHP /ACP with relevant acute hospital experience
  • Active NMC/HCPC registration.Post registration study BSc (Professional Studies in Coronary / Cardiac Care) or Experience of ECG interpretation in acute setting
  • Physical Examination Course and evidence of using these skills.
  • Evidence of professional development and regular study since qualification.

Desirable

  • Advanced Life Support (ALS) holder
  • Evidence of study at Masters Level or can demonstrate experience of operating at this level
  • Independent Prescriber (V300)

Experience

Essential

  • Minimum 6 years recent cardiac nursing experience.
  • Extensive knowledge and experience in caring for patients with complex cardiac conditions.
  • In-depth knowledge of treatment options in the management of Acute Cardiac conditions including non-invasive ventilation CPAP, Optiflow.
  • Competent to take arterial or capillary blood gasses, interpret the results and adapt treatment in accordance with the findings.
  • Experience of close liaison with members of multidisciplinary team and has developed team working skills and implemented effective teamwork across the acute or community setting.
  • Leadership experience.
