Job summary
A rare and exciting opportunity has arisen for a Case Manager in the Disability Support Team, in Dudley Rehabilitation Service (DRS). The Disability Support team is a small team of case mangers, who work along the stroke and neuro teams in DRS, and the general rehab and falls service, managing a caseload of clients of working age, with complex needs due to long term conditions or acquired brain injury (ABI), providing case management which includes carrying out holistic assessments, interventions and evaluating outcomes, in order to support them to live as independently as possible in the community.
Main duties of the job
You will be an independent practitioner managing a caseload of clients of working age, with long term complex needs including ABI, carrying out holistic assessments, interventions and evaluating outcomes, in order to support them to live as independently as possible in the community.
The DST team works closely with the specialist neuro and stroke multidisciplinary teams in DRS, to plan, co-ordinate, manage and review the care of the individual with duties including assessment, advice, sign posting and info to empower self-management. Typically areas of involvement focus around Employment, Finance/Benefits relating to Disability, Independent Living, Housing, Travel and Driving, Cognitive/Emotional Support, Specialist Equipment, Relationships/Disabled Parent Support etc. This role involves working as a lone practitioner within people's own homes and clinic settings as well as office / home-based work.
