An excellent opportunity has arisen for highly motivated, enthusiastic and caring individuals to join our Peripatetic Community Hub Team within The Dudley Group NHS Foundation Trust. You will be part of a team providing high quality care to patients across Community Teams; being able to commute for the needs of the job. The priority of the team will be to help capacity within the Clinical Hub /District Nursing teams and to provide care to patients in their own homes.
Main duties of the job
Previous experience as a Health Care Assistant in an Acute or Community Nursing setting is essential. You will have good communication skills and be able to work independently as well as part of a team.

- NVQ level 3 in Health & Social Care / equivalent experience
