We are looking for aHCPC Registered Physiotherapistwhopossessan effective communication and negotiation skillswith a desire to inspire people at all levels. As an autonomous practitioner,we expect a showcase ofyour innovative skills in the delivery of your roleprovidingeducation and training,as wellasclinical output whereappropriate withinthe Care Homes in Dudley.
Based at Brierley Hill Health and Social Care Centre, you will work within a multi-disciplinaryteam of Registered General Nurses, Mental Health Nurse Practitioners, Dietitian, Occupational Therapist and Physiotherapist.
The post holder willplay an active rolewithinthe wider Dudley CommunityHubproviding leadership,educationand support within Care Homes, with an expectation of a reduction in unnecessary,unplannedand avoidable admissions to hospital, supporting safe return of residentsfrom hospital, improvement in the quality-of-care provisionand competency levels of care staffin Care Homes in the Dudley Borough.
Main duties of the job
The post holder willprovideclinicalexpertisein the leadership and support of Care Home staff in delivering a service that helps toidentifyrisks and unmet care needs, enabling care staff toprovidean environment thatdemonstrates quality of care and safety within their facility for all residents
You will be expected to deliver education and training to care staff, as well as assist in addressing specific patient needs including review, advise,orsignpostingof theresident/ Care Hometo other serviceswhere appropriatewith theoverallaimofmaintaining their independence in the Care Homes for as long as practicable and ultimately prevention of unnecessary hospital admissionsor a safe return to their Care Homes
You will support the development of excellent standardsby means ofsupervision, education and facilitation and help equiptheCare Home staff forfuturerole expansion.
