Community End of Life Team- 30 Hours
An exciting opportunity has arisen within Community Nursing to establish our End of Life Team.
We are looking to appoint qualified Nurses with the experience of working in a role within the community and End of Life Care.
They will support the Rapid Response team which will operate 7/7 in the community, responding to patients at the end of life with acute needs to allow them to achieve their preferred place of death. The team include a band 7 Team Leader who will provide line management.
Main duties of the job
The post-holder is responsible for the on going assessment of care needs and the development, implementation and evaluation of programmes of care. The post holder will carry out all relevant forms of care and will take responsibility for caseload holders in their absence.
