Job summary
The post holder will be responsible for leading on all elements of chemotherapy delivery to the Chemotherapy Outreach Project to help deliver the strategic objectives of Macmillan and the Directorate.
Will act as key liaison person between pharmacy team and the Outreach Chemotherapy team for all chemotherapy matters
Responsible for developing guidance and SOP for the delivery of chemotherapy to the Outreach chemotherapy service.
Provide specialist chemotherapy advice to the Outreach chemotherapy team.
Work as an expert pharmacist providing specialist advice, information, support and holistic care to patients and their families about their chemotherapy treatment
Dependent on qualification - Prescribe for patients within SACT outreach service. Individuals will be supported to obtain NMP qualification.
Responsibility for development of clinical pharmacy within cancer services, in relation to both inpatient and outpatient setting.
To provide support to the Lead Cancer Pharmacist on the strategic development of pharmacy led clinic services in line with the GPhC, RPS, hospital and BOPA standards.
To support the development and implementation of medicine management strategies within the Outreach chemotherapy service, providing pharmacy input to aid in the cancer service's key performance indicators.
Main duties of the job
Development and co-ordination of pharmacy services to Cancer, including Outreach Chemotherapy service, inpatient cancer patients and outpatient clinics.
Responsibility for the pharmaceutical patient care and assessment of pharmaceutical need for patients accessing the Outreach Chemotherapy service, outpatient clinics and inpatient wards.
Monitoring of prescriptions and provision of pharmaceutical care plans ensuring active involvement of patients and carers
Ensuring the continuity of pharmaceutical care by co-ordinating drug history, prescription monitoring, clinical monitoring of drugs for patients accessing the Outreach Chemotherapy service.
Managing and coordinating the provision of pharmaceutical services including the delivery of chemotherapy to the Chemotherapy Outreach service.
Non Medical Prescriber (NMP) - Dependent on qualification
To be an independent non-medical prescriber
To prescribe within scope of practice / area of competency
To follow locally approved clinical guidelines and NICE and to be within the formulary
To provide cost effective prescribing activity
To regularly review prescribing activity and implement change as necessary
To have prescribing activity reviewed annually
To work within the framework of the Trusts Non-Medical Prescribing Group
