Job summary
An exciting opportunity has arisen for a Band 7 Registered Children's Nurse to support the continued development and enhance the Adolescent and Transition service at Darent Valley Hospital. We are seeking an enthusiastic, motivated and innovative Registered Children's Nurse to join our CYP Service to work as part of our specialist nursing team across the Trust.
The successful candidate will work across children's and adult services providing clinical, specialist and holistic support/advice to young people aged 13 to 18 years. The role will cover young people with chronic, long term and complex conditions as well as other hospital admissions for young people across the Trust.
The successful applicant will join an established service and run joint clinics in collaboration with MDT colleagues to offer the highest level of care to the patient caseload. The role will provide highly specialised support and advice. You will be expected to provide advice and support in relation to management of conditions pertinent to young people and will be expected to provide educational support to staff within the Trust.
Main duties of the job
We are looking for you to apply if you are a motivated clinical senior children's nurse, preferably with knowledge and experience in transition. You will have excellent interpersonal, IT and communication skills with the ability to organise, manage and plan your own workload effectively. You will be dedicated and passionate in your role working with young people whilst demonstrating excellent leadership and team working. You will be energetic and dedicated in working as an advocate in support of our young people and their families.

Assess young peoples health and wellbeing needs both in relation to their hospital admission, and in relation to complex or chronic long term conditions or life limiting conditions.
Plan and deliver transition programmes to address childrens complex and changing health and wellbeing needs
To support the development, implementation, evaluation of care and the setting of standards of care for young people, including those with complex or chronic long term conditions or life limiting conditions in collaboration with the multidisciplinary team.
