Job summary
The post holder will provide support to the Frailty team by coordinating the necessary interventions to ensure the flow of the frailty service. They will work alongside the Multidisciplinary Team (MDT) to ensure that effective admission to the FAU (Frailty Assessment Unit) and discharge planning are maintained in order to provide a smooth, timely patient flow and discharge from hospital. The role will also include support the team in completing the necessary clinical and administrative tasks to ensure patient flow within the frailty service.
Main duties of the job
They will be responsible for pro-actively facilitating the administrative components of admission and discharge to or from the FAU and completion of discharge documentation within their cohort of patients.
They must be able to work on their own and as part of a MDT.
The post holder will be expected to communicate with patients, their carer's and families about sensitive issues, using tact and diplomacy, ensuring a good understanding of frailty and care of older people.
Complex issues will be escalated to the Ageing and Health Coordinator and the Ageing and Health Lead Nurse

Makes sure that there is prompt initiation of investigations as per protocol to support the management of the patients by the medical and nursing staff in the clinical area.
Work unsupervised, undertaking initial assessment and agreed treatment modalities within defined care pathways, as well as undertaking work delegated by the frailty nurse specialists and senior clinical nurse specialists.
Collection, analysis and presentation of data related to patient outcomes etc
