Adolescent and Young Persons Diabetes Specialist Nurse - Transition

Job summary

We are pleased to announce the expansion of our well established Diabetes service and are looking to recruit an Adolescent and Young Persons Diabetes Specialist Nurse with a particular interest in transition. This role is a newly created post, initially for 12 months and will be working alongside the Paediatric Diabetes Specialist Nurses, Adult Diabetes Specialist Nurses, Dieticians, Psychologists, Consultants and sits within the Integrated Community Children's Nursing Team working between paediatric and adult services.

Main duties of the job

At HHFT we are always looking at developing new and innovative roles and if you have a passion for working with young people and diabetes then this will be the perfect role for you.

You will need to have a sound knowledge base of diabetes and have the skills to apply your specialist diabetes knowledge to teaching colleagues, parents and young people in the management of their care. To be able to do this effectively you will have outstanding communication and teaching skills and be able to engage your audience in what you are teaching.
