Job summary

We are looking to recruit an enthusiastic and passionate Homecare Support Worker, to work from The Countess of Brecknock Hospice in Andover, supporting our patients within their home setting.

This is an exciting opportunity to be part of the fantastic team who work within The Countess of Brecknock Hospice, caring for palliative patients at all different stages of their disease trajectory.

This is an autonomous role that requires community working, and the ability to drive, providing care for patients within their own homes.

Main duties of the job

The role of Health care Support Worker, is to assist and support patients in their own home, alongside allied health care professionals ensuring optimal care for our patients. The delivery of high quality, effective and compassionate care is essential to this role, and echo's the HHFT values.

Main duties include:

  • Providing personal care for patients in their own home, inclusive of moving and handling, basic wound care, catheter care, education to family/key members involved in care.
  • Liaising with healthcare professionals within the multi-disciplinary. team, advocating for patients within our care.
  • Supporting the inpatient unit in-between care calls.
  • Supporting the Nursing and Occupational team with aspects of care.
  • Providing support to relatives and other key members involved in the patients care.
  • Timely and prompt care delivery which includes following lone worker policy and following appropriate lines of escalation.
