The Virtual Health Hub includes the Telemedicine service, Clinical Communication Centre and Virtual Wards. This role will focus on provision of expert clinical support, leadership, and direction team, including prescribing and de-prescribing medications for patients and residents to facilitate patient safety and quality of life.
Main duties of the job
Hampshire Hospitals NHS Foundation Trust has received funding for further development of virtual health following the success of COVID virtual wards, a region wide project about empowering patients with home monitoring and expert clinical support. This allows us to continue our national leadership in virtual wards. This role will support the team with the clinical leadership and expertise across the region by upscaling Hampshire virtual care regionally to deliver sustainable transformation at Integrated Care System (ICS) level.
Telemedicine provides remote support for care home (nursing, residential and learning disability) residents across Hampshire who have become acutely unwell. It manages some of the most vulnerable people in their own place of residence and avoids unnecessary admissions as well as offering instant access nursing and medical advice for staff in care homes.
The Clinical Communication Centre acts as a single point of access to all community providers, including primary care, paramedics and district nurses. The team aim to ensure all patients receive the right care, in the right place, at the right time, by the right person, first time.
Virtual wards allow patients to get the care they need at home safely and conveniently, rather than being in hospital. The post holder will act as a clinical leader for the multiple specialities and conditions across the age range that we currently support as well as driving expansion into new areas.

- Registered nurse, pharmacist, or AHP
- Relevant MSc degree in Advanced Practice or equivalent experience.
- Independent Non-medical Prescriber
- Degree level education or equivalent knowledge and skills gained through any combination of alternative study, or employment experience
- Formally accredited physical examination and assessment skills
Desirable
- Consultant Practitioner training programme
- Leadership/Management qualification such as Mary Seacole or Elizabeth Garrett.
- Quality Improvement / Project management learning and/or development
Experience and Knowledge

- In-depth evidence of working in a specialist clinical capacity and as a senior clinical leader
